REPORTING ABSENCES

Choose one of the following methods to report an absence:

• Please call (734) 401-4900 OR
• Fill out the Woodland Meadows' Absence Form.

If you obtain medical documentation after the initial appointment, please upload documentation into the Medical Documentation Submission Form.

Please do not email Mrs. Richard or Ms. Pearson directly as they may not receive the email immediately.
